Three pathways in one compound
Older generations of compounds (such as those containing only GLP-1) worked with a single primary mechanism. Retatrutide is a so-called triple agonist — in research, it targets three different receptor classes at once:
- GLP-1 — stimulates insulin release, slows digestion, and in models suppresses appetite.
- GIP — similarly supports insulin release during meals and synergistically enhances the overall metabolic effect.
- Glucagon receptor — in combination with GLP-1 and GIP, research suggests this is key to maximizing energy expenditure and breaking down stored fat.
It's this combination of all three pathways at once that sets retatrutide apart from older molecules.
What the Phase 3 data showed
Dr. Brad Stanfield broke down publicly available data from a Phase 3 clinical trial that followed 445 adult participants with overweight or obesity (who also had mild-to-moderate knee osteoarthritis) over 68 weeks:
- Weight loss: participants on the higher dose lost an average of 28.7% of baseline body weight; on the lower tested dose, 26.4%. The placebo group lost 2.1%.
- Joint pain: among participants with knee osteoarthritis, pain (per WOMAC score) dropped by 75%, versus 40.3% in the placebo group.
According to Stanfield, these are among the most striking results seen in metabolic research to date.
Comparison with earlier generations
Compared to older preparations, retatrutide achieves significantly higher weight-loss figures. While older GLP-1-only compounds showed average weight loss of around 15% in year-long studies, and dual-target compounds (GLP-1 + GIP, such as Tirzepatide) reached roughly 21%, retatrutide's triple mechanism pushes this figure close to 29%. Sequentially adding and connecting receptors appears, based on available data, to unlock potential that a simpler mechanism doesn't reach.
